Amy Slaton Is 20 Pounds (and One Bad Habit) Away From Her Dream Surgery

Amy Slaton can practically see the finish line. After losing nearly 200 pounds on 1000-LB Sisters, the 38-year-old is closer than ever to the skin removal surgery she’s been chasing for years — and her surgeon finally gave her the roadmap. But according to reports, the green light comes with two conditions, and one of them is the habit she’s struggled with the most.
This morning we covered Tammy’s Frozen wedding dreams. But the other half of the sister duo is on her own high-stakes journey this season — and fans say it’s the most relatable story 1000-LB Sisters has told in years.
The Consultation: Amy Meets Dr. Kottwitz
In the October 13 episode, Amy sat down with plastic surgeon Dr. Kate Kottwitz — a local, Evansville-area surgeon who specializes in body contouring, breast surgery and skin removal — in a clip obtained by PEOPLE. Amy told the doctor she wants the loose skin on her stomach gone, the “apron” that’s followed her through her entire transformation.
True to form, Amy brought the humor. “If I have my skin removed, I’ll have a six pack and the rest of me look like jello,” she joked, per reports on the episode. The doctor confirmed she could help — and the consultation didn’t stop at the stomach. Amy also asked about her breasts, which she says “like to part ways.” Kottwitz told her a lift is possible but won’t add volume, meaning she’d also need implants for the look she wants.
Notably, Amy had previously consulted an LA surgeon who approved her — but the long trip would have kept her far from her kids during recovery. Going local means she could go home the same night.
The To-Do List: 20 Pounds and Zero Cigarettes

Here’s the catch. According to reports from TV Season & Spoilers, Amy got conditional approval — but she still has a to-do list before the final stamp: lose more than 20 pounds, and quit smoking entirely. The surgery can’t be finalized until both boxes are checked.
In the episode 7 DMV scene, Amy stepped on the scale at 190 pounds — remarkable progress for a woman who, with her sister Tammy, once weighed more than 1,000 pounds combined when the show premiered in 2020. Twenty more pounds sounds doable. The cigarettes are the harder sell.
Amy told the family she plans to quit smoking on Monday. Her sister Amanda wasn’t having it — she urged Amy not to wait until Monday and to quit right away. And Amanda’s concern is real: she worries more about the smoking than the weight loss, saying Amy has trouble even admitting she has a smoking problem. That’s the same smoking battle Tammy fought before her own surgery — her doctor, Dr. J. Peter Rubin, refused to schedule the procedure until she kicked her vaping habit.
Tammy Did It First — And Amy Watched It All

The irony isn’t lost on anyone: Amy has watched her big sister do exactly this journey. Tammy’s skin removal surgery was the emotional climax of the Season 7 finale, when doctors removed 10.5 pounds of excess skin — after she conquered the same smoking hurdle Amy now faces.
It’s a full-circle Slaton moment: for years, Amy watched her big sister transform while her own journey lagged behind. Now the little sister is following the same playbook — the scale, the surgeon, and the fight to quit.
Fans Are Proud — And Split
The fandom’s reaction has been a rollercoaster. Many fans say they’re cheering Amy on, proud of how far the once-bedridden duo has come. Others have been harsher, questioning her readiness on social media — some Redditors argued she still needs to focus on weight first, while longtime viewers call her journey the heart of the show.
And don’t count Amy out as a woman of many dreams: next Tuesday’s episode, “Panicking at the Disco” (October 27), has Amy stepping on stage for the first time with her music project — the AI-assisted songwriting subplot the whole season has been building toward. Surgery countdown by day, pop star by night.
Will Willpower Beat the Habit?
So here’s the Season 9 question nobody can answer yet: does Amy beat the scale AND the cigarettes? Twenty pounds is a number. Quitting smoking is a war. Tammy already proved the war is winnable — she kicked the habit and got her surgery.
Team Slaton, what do you think — does Amy finally get her dream surgery this season, or does the to-do list win again? And be honest: is the harder battle the 20 pounds, or the cigarettes? Sound off in the comments.
